Edit
Hi Lee, I am sorry you are having all of these problemswith head and neck pain. I think Anita was right on in her diagnosis of Meige and Blepharospasm. I am glad you have been referred to a movement disorder specialist. He/she should be of most help to you.

I was diagnosed with cervical dystonia and began treatment a little over a year ago. I see a movement disorder specialist in neurology. She began with Baclofen (muscle relaxer) and Artane. I took Artane for 6 months, and it helped control the neck movement. I had to quit it because of the side effects. Now I take Clonazepam at night only as well as the Baclofen. I have been on Botox for the neck for a year now and most recently Botox in the shoulders because of the shoulder pain associated with the neck movement. My doctor uses EMG to detect where the muscles in the neck and shoulders are the most active. However, the doctor is still trying to find the right amount of Botox for the best locations in the neck to control the neck movement. If overdone, it will cause swallowing difficulties.
